From Facebook:

1. Login to Facebook and click Pages in the left menu.

2. From your Facebook page Settings.

3. Scroll down and Instagram in the left column.

4. Click Link Accountand enter your Instagram username and password.

From Instagram:

1. Login to Instagram and go to your profile.

2. Tap Edit Profile.

3. Under Public Entity/Profile Information, Page.
